FSBP members, aged 18 years and older, who receives the COVID-19 vaccine or a COVID-19 booster are eligible to earn $25.

To earn the incentive, FSBP members must upload documentation (i.e., copy of vaccine card) through our secure member portal at myafspa.org, demonstrating you have been fully vaccinated. Simply log into the AFSPA Member Portal and click the new “Submit Incentives” option in the FSBP Secure Forms section. Complete the requested information and upload your documentation. FSBP will review the documentation to verify the member is fully vaccinated, and if so, will deposit $25 into the member’s Wellness Fund Account.*
